Inspired Aesthetic Medicine – Terms of Service

1. Medical Care Disclaimer

Inspired Aesthetic Medicine (“IAM”) is a medical aesthetics practice supervised by a licensed physician in the State of California. All treatments are performed by licensed healthcare professionals under the medical oversight of Leland Dao, DO, Medical Director.

All services are provided only after a legally required medical evaluation and treatment plan. No guarantees are made regarding aesthetic outcomes.

2. Appointments & Cancellations

To provide safe, high-quality care and respect all patients’ time, IAM maintains the following policies:

Cancellation & Rescheduling Policy

• A $100 fee applies to any appointment canceled or rescheduled with less than 48 hours’ notice.
• Appointments canceled on the same day automatically incur the $100 fee.

Late Arrival Policy

To ensure your full treatment time and maintain schedule integrity for all clients:

• If you arrive more than 10 minutes late, your appointment may be canceled and the $100 fee will apply.
• This is necessary because late arrivals reduce the time required to safely perform medical procedures.

Required Forms

• All intake, consent, and medical history forms must be completed before your scheduled appointment start time.
• If forms are not completed prior to arrival, your appointment may need to be rescheduled or canceled, and the $100 fee may apply.

(Rationale: Medical consents and histories are legally mandatory. They cannot be rushed or completed after your appointment time begins.)

3. Payment & Refunds

• Payment is due at the time of service.
• Prepaid services, deposits, promotions, memberships, and packages are non-refundable.
• Promotional pricing cannot be combined unless explicitly stated.
• Prices are subject to change at any time.

4. No-Show Policy

• A no-show (failure to attend without notice) results in an automatic $100 fee.
• Patients with repeated no-shows may be discharged from the practice.

5. Medical & Cosmetic Outcomes

Aesthetic results vary based on anatomy, lifestyle, and clinical factors.

• No guarantees or warranties are made regarding outcomes.
• Additional treatments may be recommended for optimal results.

6. Photography & Documentation

Standardized clinical photography is required for all injectable and laser procedures for medical documentation and continuity of care. Photos are never used publicly without explicit written consent.

7. Privacy & HIPAA

IAM complies fully with HIPAA and California privacy laws. Your information is never shared without consent.
